What We’re Looking For:
Our ideal teammate values care coordination as an essential extension of the provider’s role, helping to close gaps in patient care. As part of our incredible team of Care Coordinators, you will build strong relationships with patients through monthly calls, supporting them with services such as Chronic Care Management (CCM), Behavioral Health Integration (BHI), Principal Care Management (PCM), and Remote Patient Monitoring (RPM). You will empower patients to meet their goals, guide them on their healthcare journey, and advocate for their needs.

Our Care Management programs have been proven to enhance overall health outcomes. He applied for a grant that he was qualified for due to his leukemia diagnosis, but was denied due to his information being input incorrectly by the office he was working with. “Prior to partnering with Cosán, multiple patient requests had gone unanswered at a practice for months due to various difficulties. To better handle this issue, the provider implemented a new team on their end to field these requests.
One patient was waiting on a new shower bench, one required a Spanish speaking specialist, and one needed an appointment with a cardiologist. With the help of this new team, all of these requests were quickly taken care of and the patients received their needed assistance.”

I read him a note I’d taken during a recent Cosán meeting: “When you leave the doctor, you’re tired and have a lot of things that need to be done.” I told him, “That’s what I’m here for, to help you get those things done.”
After some thought, he replied that he no longer wanted to drop the program. This was a moment that I felt I truly had a purpose and felt that I could really make a difference in multiple patients’ lives.”

The patients I speak to on a daily basis motivate me because I see them as my family and that makes me go above and beyond to help them any way I can. My dedication to patient-centered care is always my first priority. I have spent over 20 years in senior care. I know how important ensuring independence and avoiding isolation are to the success of their overall health. I’ve also found that if I connect well with my patient, they will be more apt to share their concerns with me. I can compare their answers on their Medicare Annual Wellness visit notes with the answers on my calls, even in the same month or week, and see that they trust me enough to share things they left out during their office visit. It is an honor to gain that trust and to be their advocate.
