Nutrition Support
Nutrition support services are covered under CCM and PCM programs
Cosán offers nutrition support services to patients as part of our Chronic Care Management (CCM) and Principal Care Management (PCM) services. Working as part of the Care Management Team, Nutrition Support Coordinators have degrees in nutrition or dietetics and are certified nutritionists, health coaches, or NDTRs. Our Nutrition Engagement module focuses on:

We meet patients where they are and encourage positive behaviors for their nutrition that reinforces the patient’s ability to make healthier nutrition choices. Our approach helps patients realize that they can make nutrition choices that help them to lead healthier lives
Nutrition Information
Our Nutrition Support Coordinators work with your Care Management Team to provide individual support on patient-elected topics concerning nutrition. We collect that input directly from the patient to understand current eating habits, set SMART goals (Specific, Measurable, Attainable, Realistic, and Timely) and provide continual encouragement to the patient to achieve their goals.
Educational Guidance
Nutrition guidance is disease-specific, delivered in a manner that is easy to understand to help patients make realistic goals and attain them. It’s designed for long-term, sustainable success.
Our goal is to support patients in their journey to realizing that nutrition change is feasible, and with even small and consistent changes over time that it can help them lead healthier lives.
