Cosán’s unique approach to Behavioral Health Integration and Monitoring (BHI) helps us better identify a wide range of issues for early intervention. We perform regular stepped screenings to the entire patient population enrolled in care management services.

While many programs cover a narrow set of conditions, Cosán extends this coverage to additional behavioral, mental, and social conditions negatively affecting patient outcomes.

In coordination with the overseeing provider, Cosán assists in validating affected patients and coordinates a care plan to support treatment. These plans are structured to help proactively manage the patient’s mental well-being through delivery of behavioral health-focused care coordination services.

Our dedicated nursing staff of RNs and LPNs, who have a background in clinical treatment of behavioral issues, develop a trusted relationship with patients and work with them each month to improve their condition state. Where beneficial, additional external resources and non-clinical support like arranging transportation or providing condition-specific nutrition ideas are integrated into the patient’s care plan.
