These calls with the nutrition support coordinator are offered as part of the Chronic Care Management (CCM) or Principal Care Management (PCM) service which you are already enrolled for with your provider.
These calls are scheduled separately from your normal care coordinator to allow a focused conversation on diet and nutrition.
