RPM uses digital tech to remotely monitor patient vitals in real time

Cosán’s Remote Patient Monitoring (RPM) service aligns with our condition-specific care plans for each patient. Developed and refined by Cosán’s clinical and informatic leadership in conjunction with existing client ACOs and health systems, Cosán’s care teams track the progression and management of illness against condition-specific protocols to support positive health outcomes.

This program employs patient-utilized devices to monitor and report on various physiological readings. The data collected is then leveraged by our clinical support staff to:

  • Provide early warning to providers and care teams of potential issues.
  • Increase patient engagement with their care plan.
  • Promote better health outcomes through proactive management of the patient’s chronic conditions and patient-driven interventions.

Cosán believes that for RPM to be successful, reliable, accurate, and easy-to-use devices must be made available to patients, regardless of the targeted condition set.

With this goal in mind, we’ve partnered with industry-leading medical device manufacturers who provide robust and scalable monitoring equipment. All our devices are 4G cellular-enabled and directly connect to Cosán’s care management platform.

No phones, tablets, gateway hardware, or home WiFi is required, enabling a compact and always-connected solution regardless of the location.
